Updates to Visit Documentation Policy

Effective May 1, 2020, the Visit Documentation policy changed from requiring the SOAP format (Subjective, Objective, Assessment, Plan) to instead requiring any format which contains all the elements of SOAP, such as APSO (Assessment, Plan, Subjective, Objective) .

This policy is intended to decrease administrative burden by giving providers greater documentation flexibility.

  For Dates of Service Prior to May 1, 2020 For Dates of Service On/After May 1,2020
Required Note Format SOAP Any format arrangement which has the subjective, objective, assessment and plan elements.

 

The Outpatient Speech Therapy Visit/Encounter Note section has been revised to match the Outpatient PT/OT policy for documentation. Details can be found in the Physical and Occupational Therapy (PT/OT) and Speech Therapy billing manuals.

Self-Mobilization via Telehealth Course

As we continue to navigate through the interesting times of COVID-19 and physical therapists are emerging into telehealth in a big way, many clinicians struggle with not being able to put their hands on their patients. We have heard from many clinicians across the country about their challenges and as manual therapists ourselves have successfully made this transition and have been operating at a full schedule with great patient satisfaction. To help others with utilizing telehealth to encompass the depth and breadth of our skills as manual therapists, the 10 Doctors of Physical Therapy at Rezac PT have put together a 5 hour course covering 9 sections to instruct clinicians in teaching their patients self-mobilization techniques, which we would typically teach patients to complete during in-person sessions, but now adapted to a the telehealth setting.
