Please submit your nomination below for the APTA Colorado Chapter Outstanding PT and PTA of the Year Awards! The Outstanding PT and PTA of the year is a chapter member that demonstrates Excellence in an area of practice (clinical, administration, research, and education) and has made significant contributions to the profession, the community, and the chapter. All nominees must be current members of the APTA Colorado Chapter.

Submit Your Nomination for the 2024 APTA Colorado JEDI Leadership in PT Award!
Creating a more inclusive physical therapy (PT) profession will require practitioners to embrace the principles of justice, equity, diversity, and inclusion (JEDI). The application criteria for the APTA CO JEDI Leadership in PT Award include the following: 

  1. Evidence of significant contributions that advance awareness and understanding of issues related to justice, equity, diversity, and inclusion within the field of physical therapy.
  2. Evidence of leadership and collaboration in promoting justice, equity, diversity, and inclusion within the field of physical therapy and society at large.
  3. Evidence of efforts to promote inclusive and welcoming environments for patients, community members, learners, and other individuals from diverse backgrounds. 
  4. Evidence of the nominee's ability to inspire and mentor others in promoting justice, equity, diversity, and inclusion within the field of physical therapy.
  5. Evidence of how the nominee's work aligns with APTA's core values, including altruism, collaboration, compassion, professional duty, inclusion, and social responsibility. 
  6. Evidence of how the nominee’s work advances APTA's mission (Building a community that advances the profession of physical therapy to improve the health of society) through justice, equity, diversity, and inclusion.
  7. Evidence of sustained publications, presentations, or other outcomes that demonstrate the nominee’s commitment to justice, equity, diversity, and inclusion within the field of physical therapy.
  8. Must be a current PT or PTA member of APTA Colorado.

Award Nomination Instructions:
The application package should include the following:

  • Nomination letter (2 page max)
  • Nominee's resume or CV
  • 2 letters of support
